04 x 5 = 20

Add +6                                                           

20 + 06 = 26

Multiply by 4                                                   
26 x 4 = 104

Add +9                                                               

104 + 9 = 113

multiply by 5                                         

113 x 5 = 565

Add date of birth                                   

565 + 05 = 570

Now tell the answer you got

Date is 05

Month is 04

Subtract 165 from the number told

thats it

If you get 4 digit number then answer is first 2 digit is Month and Last 2 digit is Date.

If you get 3 digit number then answer is first 1 digit is Month and Last 2 digit is Date.

From the above example

Number said is 570

So,

570 - 165 = 405 

Here 4 is April Month & 5 is Date.
